A sworn statement is a statement that may be requested of you if you have no other means of proving something about yourself. This is a letter that you will sign in which you declare your good faith and certify that a situation concerning you is true. You should not write it by hand unless instructed to do so first. Be careful not to make a false statement, it could be used against you in case of disagreement or problem. You may, for example, be asked for a sworn statement at the time of a PACS or marriage, when setting up a business or even to register with Pôle Emploi.
The sworn statement must be written on plain paper. At the top left, enter your first and last name and your address. You can also add an email address and a phone number although it is not required. Opposite your contact information, at the other end of the letter, include the recipient's contact information. Name, address, e-mail and telephone number. Just below, indicate where you wrote the letter and the date, for example:"In Paris, January 12, 2015". Go back to the line on the left of the letter and write:"Subject:sworn statement". Return to the line and start the letter. It can be written in the form:"Madam, Sir, I, the undersigned (indicate your surname and first name), residing at (indicate your address) swear to (indicate the fact or the situation that you certify). I am aware that a false certificate may lead to criminal penalties and certify the veracity of the facts attested above. Return to the line and indicate "Made to serve and assert what is right" then sign at the bottom left of the document.
